Insurance Rates and Information
If you plan to use health insurance benefits to help cover the costs of therapy services, fees are based on the contracted rates for your health insurance plan and are different for each health insurance company. All copays, coinsurance, and deductibles are collected at the time of service using your credit card on file. It is your responsibility to contact your health insurance provider before your first appointment to determine your coverage and out-of-pocket expenses. You can check your benefits by calling the phone number on the back of your health insurance card.

Please note that in order for insurance to pay for appointments, a diagnosis must be given and medical necessity for therapy services must be established.
